00:00:00: Hello and welcome to Dubai Dogs' Diary, your podcast where Dubai dogs telling you about their journey in the healthcare sector.

00:00:28: Today's guest is a surgeon who operates at The Intersection of Modern Surgery Aesthetics Regenerative Medicine & Longevity.

00:00:39: As a labroscopic aesthetic and regenerative surgeon Dr.

00:00:44: Eshan combines minimally invasive surgery techniques with cutting

00:00:50: edge

00:00:51: regenerative treatments designed to improve not only how a patient feels, but also how they feel and function.

00:01:01: In this episode we explore the future of medicine anti-aging aesthetic longevity and the rapidly growing role of regenerative therapies in health care.

00:01:18: So Dr.

00:01:18: Ishan, if somebody meets you outside of medicine how would you introduce yourself?

00:01:24: And who would say that are what your doing ?

00:01:30: I like to someone knows me as some expert physicians i hope so.

00:01:36: a little bit playing music.

00:01:39: Oh, what music you're playing?

00:01:42: I'm playing one traditional musical instrument from my country.

00:01:46: Oh

00:01:46: really

00:01:47: look like of the guitar or a little different.

00:01:49: What's it called?

00:01:50: The name is Tahr in international language.

00:01:53: Yes for my Iranian friend she also talks about this instrument.

00:01:57: i am very excited

00:02:00: and its shape Very nice, this look like of the heart shape.

00:02:07: Next time I will bring it

00:02:08: Next time you're playing it.

00:02:10: Dr.

00:02:10: Ishan, but today we don't want to talk so much about music.

00:02:13: But your guest of Dubai Docs Diary and as such We wanted to talk about Your daily life is a doctor here in Dubai.

00:02:22: So what brought you first?

00:02:24: To the field of medicine at all?

00:02:27: And What was your motivation?

00:02:29: You initially Yeah Reason why you have explored the medicine and finally made it to your field.

00:02:38: At the first, as you know I am a minimally invasive surgeon.

00:02:41: I'm a laparoscopic surgeon more than twenty-five years old.

00:02:46: but because from some years ago i understand that something will be happening in the medicine.

00:02:52: I am joining to also the regenerative medicine...I take the certification of the regenerative medicine for American board And right now, I am more focusing in the regenerative medicine because i think we can use from our body power.

00:03:10: From our physiology to repair a lot of disease.

00:03:15: Right Now In Eden Esthetic Clinic...I'm doing multiple different actions like for example patient fat as a non-fat or micro fat for rejuvenation, regeneration and also contouring of the face and hand on some other area.

00:03:32: I am using the advanced PRP that we are taking the plasma off the patient And then uh... We can use this plasma for multiple different purposes.

00:03:44: Also, I am very interested and working on the peptide field.

00:03:48: As you know, peptides right now is boosting a lot And peptide is one core of longevity.

00:03:55: For this reason we are working in the Eden Clinic on the longevity fields with the peptides and with stem cells and exosomes.

00:04:04: Right at first it's very powerful.

00:04:08: All of them have some natural techniques for not only treatment, but also for the protection of some disease and repair of damage on longevity purposes.

00:04:23: I think future is belonging to biomolecule.

00:04:27: as you know right now we have medicine that approximately cancel two-thirds all surgery for obesity.

00:04:34: The name is Monjaro or Neogeneration is Rototrotide.

00:04:39: We are using this type peptide, this is one type of the peptide.

00:04:44: And effect and efficacy are sometimes more than surgery... ...and it's a miracle effect!

00:04:51: Yes I mean all these specifications.

00:04:54: i want to go deeper into all these fantastic techniques and opportunities that we have.

00:05:02: take care our body in terms of longevity.

00:05:06: but Let me pick your brain a little bit.

00:05:09: What attracted you to become a doctor in the first place?

00:05:14: when you was in your home country, obviously then there must be having.

00:05:19: There must have been an initiative moment where he said okay this is the field that I wanted to explore.

00:05:25: and back in the days they were not these technologies there They will not this opportunity.

00:05:30: so what has?

00:05:32: Been the initiation of Your first career

00:05:36: initiation because i love the physiology at.

00:05:42: I was interested how the body work.

00:05:44: After that, step by step... ...I'm joining to the medicine.

00:05:49: my course in the medicine in general practitioner is it was eight years after that straight- I am going through surgery because i love the surgery and I passed a course more than four years in this field.

00:06:03: And then I start practice especially minimally invasive.

00:06:10: right now in our surgery, more than ninety percent we are doing only laparoscopy surgery.

00:06:16: In the laparoscope recovery is very fast for a patient and pain compared to open surgeries it's less.

00:06:26: We can do all techniques that we're doing with the open surgery with the Laparoscopic.

00:06:32: This is the efficacy on power of technology which has this facility right now.

00:06:39: And I'm interested back in the days, when you became then a doctor and when you had taken decision.

00:06:47: Most of people they are sticking with one specification but do have more than what?

00:06:54: You have three specifications right?

00:06:57: Honestly i love research.

00:07:00: Right now for example today I have on research team that joined me after nine o'clock to working on the autism as a research project with the peptide and some antioxidant material.

00:07:18: We believe that a lot of disease is possible to treat it by this type of technique, we are not only working in the sign and symptom but also at the core of the disease.

00:07:34: knows that this technique, how much is the powerful?

00:07:39: Unbelievable effect.

00:07:42: I am talking about the peptide and I'm taking about stem cell.

00:07:46: as you know a stem cell is mother of the cells And has an ability to producing some material to push the body for healing or recovery and reduce inflammation.

00:07:58: I think future belongs to this type of the technique.

00:08:01: That's what that was my first next question, where you think the medical field in especially your specification is going to lead to everybody talking about longevity and everyone wants to have this even i think it sometimes misused by a buzzword.

00:08:22: but uh when do we need technology opportunities to treat our body for a lifespan-extending way.

00:08:32: Exactly,

00:08:33: longevity in the longevity we are not thinking about that time We're also thinkin' about quality of life.

00:08:40: I mean longevity is not only how many years were living it means How much is the quality of their living?

00:08:49: And we are focusing in this type of treatment.

00:08:52: At first, we evaluated the patient off course taking a chief complaint or present illness and then going to doing some blood test, some scan of the tissue that showing without any invasion.

00:09:06: How much is there vitamin deficiency and so on?

00:09:09: And then we are focusing on the IV injection as a for example NAD.

00:09:15: right now it's very trend.

00:09:17: NAD plus yes.

00:09:17: Yes.

00:09:19: Nad plus is working as fuel after sales.

00:09:22: also recovery and detoxification.

00:09:24: Then you're working in the anti-oxidant.

00:09:28: why were going to old age?

00:09:30: because of the oxidative esters.

00:09:32: and if we reverse this bad effect, We can recovery make a recovery our body.

00:09:38: Then we are going to do multiple different type of the peptide that these types of the pep tide each have one special effects.

00:09:46: some of them is for recovery Some often it's for longevity also.

00:09:50: So sometimes reduction of inflammation Sometimes working as a neuro protection like Reduce stress anxiety.

00:10:00: Some of them make a correction after sleeping time.

00:10:03: A lot of good effect without, approximately with outside effects.

00:10:09: and then the last generation most powerful technique that we have in the Eden Clinic from some years ago till now is the stem cell mesonchymal stem cells and also exosome data.

00:10:21: both effect like a multiple different peptide.

00:10:26: I mean stem cell working as the factory of the creation of the peptide in our body.

00:10:32: and when we are kids, if some laceration happened or some broken happen very fast recovery at eight years old no any recovery.

00:10:43: why?

00:10:43: because at the eight-years-old approximately each two million cells have only one Stem cell is mean zero, approximately.

00:10:51: But if we can reverse... If you increase our body's stem cells Our recovery or healing will be jumping up and also at the same time inflammation going down And with this technique We receive more longevity and quality of life.

00:11:09: Dr Ishan when I got to know your first like four years ago told you one time I wanted to become one hundred and twenty, i don't know if you still remember.

00:11:22: And that's when I have also received once an NAD plus infusion.

00:11:28: so...and uh..that is my big plan!

00:11:32: Just had a birthday happened and got a book called the Blue Zones.

00:11:39: You notice?

00:11:40: The book actually saying something about.

00:11:45: You mentioned stress, that the place where you live is very much responsible for how good your stress level and longevity are.

00:12:00: longevity is not only to increase the lifespan, but also the life quality.

00:12:06: And we all know how challenging Dubai is in terms of performance and it can be stressy if you don't manage your schedule or say no outside an ASTU then a lot of people cannot deal with that.

00:12:26: how you think, for example in this book if you might not know then it's actually saying that see a stress and positive environment is also responsible.

00:12:43: people who are growing very old in this.

00:12:46: For example, and this book was written that Sicily has is one of these blue zones because it has overestimated beyond the average.

00:12:57: actually a lot of people become one hundred and ten, one hundred twenty or something like that?

00:13:03: And they have discovered are existing in different kind of the words Sicily, California.

00:13:13: I think there's one increase.

00:13:15: How do you think Dubai has to potential?

00:13:19: To be also such a blue zone In terms of longevity and become actually The place-to-be especially when it comes to technologies to combine with a place nearby the sea, in weather and positivity.

00:13:41: Now I imagine to combine this with your technology... I mean tell me!

00:13:48: Is Dubai having actually the potential to be also a blue zone?

00:13:54: Absolutely.

00:13:56: Dubai is one of the pioneers in the world, for example right now as you know a lot of countries have no facilities like stem cell and exosome treatment but we some years ago, and most of the biggest conference related to the longevity and regenerative medicine happening in the UAE also.

00:14:20: Yes I believe that this is very good area for growing in these fields and I think future belongs to this technique.

00:14:29: We are not only working on aesthetic field as a For example, advanced technique of the hybrid filler.

00:14:38: Advanced and newest techniques of the booster.

00:14:41: that right now is very trended And I believe this better than traditional treatment Of aesthetic.

00:14:48: we are also working on core body.

00:14:51: I mean for totally health longevity well-being and quality.

00:14:57: Imagine how many people coming after the trauma, after their post-exercise pain or trauma to the tendon and with treatment of a very simple peptide completely recovered.

00:15:11: Or after injection of the stem cell... How many people recover?

00:15:17: Especially that people who have autoimmune disease.

00:15:20: because as you know the stem cells has an powerful technique for reduction in inflammation.

00:15:27: Also for esters we have the same technique as a peptide without side effect, without dependency.

00:15:35: Without the drowsy side effects after using.

00:15:41: and they are neuro protector support the brain.

00:15:45: I believe that future belong to UAE especially right now when we have multiple different laboratory that is specialized in this techniques high quality to prepare enough material for us, like advanced stem cell advance PRP Advanced Exozo.

00:16:07: You mentioned that Dubai is a pioneer.

00:16:10: Plays for also giving the permit even use steam cells.

00:16:16: so we are seeing lot of medical tourism coming from here.

00:16:19: Exactly!

00:16:20: We have lots of patients who come from other countries.

00:16:24: exactly this treatment

00:16:26: I have to a specific disease, some kind of if you allow me too.

00:16:32: Absolutely because as for myself on lipidemia and i have researched that A lot of Lipidemia cases came since the fifties sixties And even more now in The ninety twenties.

00:16:47: it was just not discovered is hasn't Even this existed or Just somebody didn't know about It?

00:16:54: but nowadays every The second ninth female is actually suffering of lipidemia.

00:17:00: Would steamsess, for example have like that or with that as well?

00:17:04: And how

00:17:04: would you treat this?

00:17:05: You mean hyperlipidemia.

00:17:07: about the hyperlapidemia we believe That This Is Some Metabolic Disorder.

00:17:12: at first We Are Not Focusing on the only hyperlepidemia.

00:17:17: At First We Must Be Exclude Or Rule out Of insulin resistance.

00:17:23: Most of the patient that has an insulin resistance, I mean maybe your blood sugar is normal even you're Insulin level in the normal range but the balance between to the sugar and insulin is abnormal.

00:17:38: This is insulin resistance And most of case with treatment of the insulin resistance also hyperlipidemia will be controlled.

00:17:48: This is the art of the regenerative medicine that we are not focusing on one disease like a keyhole.

00:17:56: We're looking whole body, we evaluated human as even completely live organ.

00:18:04: for example in this life organ esteris is an important thing.

00:18:10: The other insulin resistance will be leading to hyperlipidemia working on treatment of the otteroskelerosis in our heart.

00:18:19: We believe that if we can do the treatment for insulin resistance at a young age, then we will protect patients from old-age otteroeskeleroses and right now we are not waiting only to see how much sugar there is.

00:18:39: treatment of the insulin resistance before the patient receiving to the diabetes type two.

00:18:45: And that is something I think it's so good.

00:18:48: when...I'm working with doctors here, help them come from the European side too hear and enter market with licensing as well in this time.

00:18:59: uh..of course they look for a job.

00:19:01: and When do you present their cases?

00:19:03: When did you present there questions or work?

00:19:06: They totally have a different portfolio of operations, of treatments that they used to because here in Dubai there's the total other landscape diseases or needs to treat things.

00:19:23: A lot people which makes this country so good for longevity out my point if you correct me if I'm wrong.

00:19:29: but we don't wait until we are sick starting already before.

00:19:37: so preventive medicine is something very big and very good established here in the society, right?

00:19:46: And I imagine steam cells could be a big part of the prevention.

00:19:54: One of the most powerful not only for longevity and well-being.

00:19:58: also we are using the stem cell even for aesthetic purposes.

00:20:03: We can use stem cells as a booster of the skin on their face or hair growing, at same time we can also use this type treatment to damage the knee after trauma and only for longevity purposes.

00:20:20: Also for neurodegenerative diseases that till now confirmed treatment or approved treatment, we can use of the stem cell for a slow growing of disease and reverse some bad effect on chronic diseases.

00:20:38: I mean our job is prevention.

00:20:41: you are completely right not only focusing on treatment.

00:20:44: Right now we believe that prevention is much better than the treatment.

00:20:50: And after COVID suddenly working on this type of the biomolecule and stem cell, and peptide.

00:20:58: Suddenly all the world growing a lot because often necessary happen in some demand happened after the COVID.

00:21:08: Yeah that's totally interesting for me.

00:21:10: also when somebody hears this from Europe they are having the motivation to come here whether as patient or also as a doctor, because it can be very attractive to be allowed to work something that you're not permitted in different countries.

00:21:27: So what do think is the biggest mistake of for example?

00:21:32: A patient can jump into the plane coming here... Do they expect, basically from you to make a magic in one or two weeks?

00:21:46: How is it going actually.

00:21:47: No of course there's no any magic but always I am waiting for.

00:21:53: our plan in the Eden is this one that patient if want to come into Dubai for their longevity purposes Or using of the stem cell before anything we have some consultation online so i can see the patients feet can take enough benefit.

00:22:11: There is no, for example any underlying disease that cancel this type of the treatment.

00:22:17: all of them evaluated and also if need any blood test we ask to do it in their country.

00:22:23: then If they are fit We arrange everything for the patient coming here.

00:22:27: hotel is ready ticket is ready And before the coming year we arranged everything.

00:22:32: I know some patient coming only for short time.

00:22:37: We arrange all of them in the short-time to receive the maximum effect, Of course always we advise to stay more so that we can completely our plan.

00:22:46: but In some rare case also we can do even four or five days.

00:22:51: The maximum thing is that you can't do it.

00:22:54: For this reason Always like before a patient make plans and have consultation with him.

00:23:00: Okay, that's very good and it is very effective I must say.

00:23:04: And how many times they have to repeat some kind of for example a steam cell?

00:23:10: This totally depends on why we are starting the stem cells.

00:23:16: if you're starting themself or only longevity and protection We can use one time per year two-time period.

00:23:23: but If the patient has an underlying disease like poor control diabetes like for example, protection or starting the sign and symptom of Alzheimer Parkinson's.

00:23:34: Or some neurodegenerative disease even we can repeat four times per year in very rare case that this is different story.

00:23:43: For example admitted into hospital on a critical condition Even if you can't repeat every week but it is very routine leave using one to time.

00:23:53: That sounds reasonable

00:23:56: and also manageable in terms of traveling.

00:24:04: spoke to a colleague from any other country who wants to jump into this medical landscape here where they can explore, treat patients on that sort of treatment especially when it's not permitted in another countries.

00:24:22: What would your advice be for those colleagues?

00:24:27: what could you do wrong and what must I write?

00:24:30: At the first, we must be categorized.

00:24:32: The patient.

00:24:33: which type of disease is better response to the regenerative medicine that the first category is autoimmune diseases and some diseases has a that have an inflammation in their body?

00:24:48: longevity purposes.

00:24:49: Always, I am asking to do the consultation before transferring the patient too.

00:24:54: we can arrange everything and choose the best patient for this type of treatment because these types of treatments if doing correctly has a lot benefit from the patients.

00:25:08: Yeah so you were talking about PRP.

00:25:12: Yes,

00:25:13: PRP.

00:25:14: So

00:25:15: what you think is the most effective treatment that you would say today?

00:25:21: Is basically the most futuristic treatment as well where you can have the most out of it.

00:25:28: people are asking...

00:25:30: Stem cell.

00:25:31: future belong to the stem cells but I am sure that a stem cell also going through another category different type to some of them specialized for specific treatment, a lot of university working in this field and I think future belongs to these type of the treatments.

00:25:49: And every day you can listen new medicine that has an amazing effect all related with biomelecule or bio-technique.

00:26:05: It's amazing effect.

00:26:07: Exactly for your information, Monjaro or rotatrotide.

00:26:12: Nobody believed that this medicine simple medicine.

00:26:16: how much is the affect?

00:26:17: right now we can treat it.

00:26:19: that diabet type to fifty percent cure its means finished completely and Approximately a surgery for obesity going to the memory.

00:26:33: Not using it at all.

00:26:34: Going

00:26:34: into the memory, yeah?

00:26:36: I think this is such an big advantage also for humanity.

00:26:40: basically uh...to have Manjaro?

00:26:43: because Yeah!

00:26:44: A lot of diseases..a lot of crisis within your body are actually the root cause by being obese.

00:26:52: right.

00:26:52: so

00:26:52: Right.

00:26:53: but one thing that i like to mention from this field using only Mungaro without support of the expert has a side effect.

00:27:04: I saw lot people that, with out indication use the Mungaroo and also they are going to their muscle loss.

00:27:11: this is not good at all.

00:27:12: we have some technique.

00:27:14: We can add different type of peptide.

00:27:17: Also same one happening for stem cell or exosome patient coming ask okay Use the stem cells but need expert evaluated to see the best technique, which techniques more have a benefit for the patient.

00:27:34: So how is that treatment going with steamsays?

00:27:36: For example if somebody never heard about this they are interested in prevention and what they're doing... Is it an infusion or injection?

00:27:50: exactly all of them as possible.

00:27:52: infusion for different treatment, different method.

00:27:57: Injection as a local for treatment of the recovery and healing of damaged organ like tendon or joint and also injection on the skin to protect scar or treatment from the scar for aesthetic purposes is one of most powerful techniques right

00:28:18: now Okay.

00:28:20: So, yeah that sounds manageable.

00:28:23: also to integrate it in a daily life right?

00:28:25: That's how?

00:28:26: yes of course and this is Also from your own body.

00:28:31: we are taking some fat from the patient And then extract the stem cell off the fat From the patient again.

00:28:40: It makes you culture for million-million times and patient has access to this pulling of the stem cell for lifetime, unlimited time, unlimited number.

00:28:54: This is for any type of treatment.

00:28:57: Now I am always a fan having something that people can relate too.

00:29:03: Tell us about your most remarkable heart feeling transformation you have guided and created with patient Which you

00:29:15: mean about aesthetic

00:29:16: or like kind of a case where it was difficulty.

00:29:20: and how did you watch the patient?

00:29:24: How they transformed their lives.

00:29:28: And so, we can go away from only designs background towards human stories that are behind cases

00:29:41: right now.

00:29:41: I can see in comparison to some years ago, I can see a lot of very educated patients because they have Social media, they are reading a lot coming specifically and very correctly.

00:29:59: Asking some technique for the treatment right now.

00:30:02: trend going to them more natural techniques.

00:30:05: For example Right Now Trend Going From The Filler To The Bio-Filler.

00:30:10: Trend Going from the booster of the skin to the bio stimulator.

00:30:15: that effect is much better and long-lasting.

00:30:19: We have a lot of different bio stimulator that is more natural, leading to the longer term youthfulness also same one happening for inside of the body.

00:30:30: A lot patient coming in ask me about peptide.

00:30:34: some information on education.

00:30:38: Excellent.

00:30:39: I mean, they have all the information related to the old type of the peptide and also for a stem cell.

00:30:44: some patient right now coming in is amazing For me that They
